The Community Occupational Therapy team may be able to help if you are have difficulty managing with daily living activities due to illness or disability.
They can :
- support people to remain independent in their own home
- promote independent living through appropriate equipment and adaptations
- assess and advise on rehousing for people with disabilities
- carry out Independent Mobility Assessments (Blue Badge Scheme)
- advise on Moving and Handling for family carers / non paid Carers

Request an assessment by the Occupational Therapy team
Once we get your referral, we'll look over the details and check if you qualify to see if our service can help you.
Referrals are prioritized based on the information you give, and we will contact you soon to schedule an appointment.
Assessments where possible are carried out at the Link Centre within our specialist activities of daily living suite. Home visits are possible in exceptional circumstances.
Mobility
The Community Occupational Therapy Team does not provide wheelchairs, please request a referral to Wheelchair Services via your GP. Alternatively, you may be able to rent or buy wheelchairs from suitable suppliers.
If your referral is requesting support with walking and overall mobility, including the risk of falls, please discuss a referral to Physiotherapy with your GP.
