Changing Futures is a service with the aim of improving outcomes for adults experiencing multiple disadvantages.
The team at Oldham will work with adults experiencing three or more (can be combined) of the following:
- Homelessness
- Substance misuse
- Mental health issues
- Domestic abuse
- Criminal justice
The service will work together with local partners to improve outcomes for this group.
Our team includes six adult support workers, two of which will work with women only, and a team manager.
How do I make a referral?
*Please note, we can only accept referrals for this service from professionals – this includes the voluntary sector. Professionals can contact the team via email or by attending the Making Every Adult Matter (MEAM) meeting.
